There is no direct relationship between our office and your medical insurance company. The type of plan chosen by you and/or your employer determines your insurance benefits. As such, we have no say in the selection of your insurance company, no control over the terms of your contract, the methods of reimbursement or the determination of your insurance benefits. Therefore, we do not accept assignment of benefits from any insurance company. Any reimbursement by your insurance company should be made directly to you according to the terms of your contract with them. As a courtesy, we will provide all necessary documents to assist you in filing a medical insurance claim.

Treatment costs are determined following comprehensive diagnosis and are customized for each patient.  

Symmetry requests payment at the time of service.  Accepted payment methods include VISA, MasterCard, American Express, Discover Card or checks. Interest-free programs or traditional longer-term financing is available through CareCredit. We will provide information for you to research, compare and determine which plan might be best for your situation.

Dr. Deal has opted-out of Medicare.  By law we may not file with Medicare for any services provided in our office or provide you with the means to do so.

Night guards reduce chewing surface wear only. They are not designed to address jaw muscle and joint dysfunctions.

Rarely does dental or medical training include instruction regarding diagnosis or treatment of TMJ.  Due to the numerous seemingly unrelated symptoms, TMJ is often misdiagnosed.
